PLEASE READ CAREFULLY: YOU NEED TO COMPLETE ALL OF THESE FORMS TO PRACTICE OR PLAY ON ANY SPORTS TEAM!!
Complete the attached Athletic Folder and submit to the Student Store at lunch and nutrition ONLY. DO NOT TURN THE FOLDER IN TO YOUR COACH!!!
You need ONLY ONE FOLDER per school year (includes Summer 2013), You will need a separate Emergency Medical Card for EACH SEASON YOU PLAY!!! (Summer, Fall, Winter and Spring)
An annual physical examination or a statement by a medical practitioner certifying that the student is physically fit to participate in athletics is mandatory before a student may try out, practice, play or participate in interscholastic athletic competitions and is included in the Folder. CIF-SS Bylaws, Sec. 306. THE PHYSICAL MUST INCLUDE: *the date of the actual physical exam, *the examining physician's signature and *legible printed name, and the physicians *OFFICE STAMP. Immunization records and payment receipts are not acceptable.
*FOR THIS YEAR, CHS recommends that the annual physical be taken after May 31, 2013, so it will be valid for the entire school year.
EMERGENCY MEDICAL CARD
**You need a separate Emergency Medical Card for EACH SEASON of the year the athlete plays (Summer, Fall, Winter and Spring)! Cards cannot be transferred from one sport to another! Once your folder is completed and received by the Student Store, your Emergency Card will be stamped and returned to you to give to your coach. This stamped card is your ticket to participate, and will be kept by your coach for the duration of the season.
Our sports programs are dependent upon the generous financial support of our parents. Without your contributions, especially with the current fiscal crisis in education, athletics at Calabasas High School could not exist. While no child will be denied the opportunity to participate based upon the inability to contribute to the program, we are requesting the following donation to help offset the cost of running an athletics program at the school. This tax-deductible donation will be used to defray team transportation costs, for partial funding of an athletics trainer, and maintenance of the athletics' facilities.
