FIELD TRIP PACKET - the following forms are included in this packet. Fill out the ones you need and turn them into Lisa Solomon in the Admin Office to get AP approval and to order transportation.
1) Field Trip Request Form that needs to be submitted for AP Approval
2) Field Trip Notification Form for students to get signed by teachers
3) Parental Permission Form for approval and medication needs, if any
4) Durham Bus Request Form if transportation is required
5) District Vehicle Request Form if district van is required.
